Volunteer Application, Confidentiality Agreement, and Release of Liability
This document represents an agreement between The Grand Rapids Red Project (GRRP) and the participating individual. The participating individual confirms that the information on this form is their correct personal information and that the completion of this form acts as an electronic signature.
Confidentiality
Respectful and strict confidentiality is expected of all volunteers. All observed or communicated information, whether obtained formally or informally, is to be considered completely confidential - even if it seems public or unrelated to GRRP. Names and identities of clients are also confidential.
By participating as a volunteer, I agree to respect the privacy of other volunteers, staff, and clients and not to disclose anything that is discussed during hours of service, before or after hours of service, or during breaks from hours of service. If at any time I violate the confidentiality of another volunteer or client, I understand that my participation with GRRP is terminated and I will face the full legal consequences of my actions up to, and including, fines and imprisonment.
Release of Liability
I hereby release GRRP, agents of GRRP, and clients from any and all liability for real or perceived physical, mental, or emotional harm incurred before or after, during travel to or from, during breaks of, or during, the hours of service.
In consideration of my participation in any and all events of GRRP, I, for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages, demands, actions whatsoever in any manner, as a result of participation in any activities/events including travel to and from events.
By completing this form and submitting the information below, I agree to the conditions herein.
Consent, Photography Release, and Video Release
Informed consent for The Grand Rapids Red Project participants and volunteers.
I hereby certify that I have received and read the consent form, or that it has been read and explained to me, and that I understand its content. The submission and completion of this form indicates that I freely agree to participate and/or volunteer with The Grand Rapids Red Project at this time.
I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video devices without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational setting within an unrestricted geographic area.
Photographic, audio or video recording may be used for the following purposes:
- Conference presentations
- Educational presentation or courses
- Informational presentations
- Online information presentations
- Print materials
By submitting this form, I understand this permission signifies that photographic, audio or video recordings of me may be electronically displayed via the internet or in the public educational setting.
There is no time limit on the validity of this release, nor is there any geographic limitation on where these materials may be distributed.
By completing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.
The Grand Rapids Red Project does not discriminate on the basis of gender, race, religion, age, country of origin, sexual orientation, gender identification, political association, drug use, or physical change.
